PK5 Sample Web Feedback Form


This sample web feedback form illustrates basic HTML coding options for students who wish to create on-line research questionnaires or otherwise solicit responses which will be e-mailed to the address specified by the student in the form. The forum uses the UTA Mailaa program, brief documentation on which is also available in Finnish. Use the "view source" command to see how the appearance and action of this form has been implemented, as a guide to how you would like to create your own forms.

Note that information sent via this form will be anonymous unless the person completing the form specifically gives his or her name and e-mail address. In your e-mail "inbox," responses will be be described as coming from "Anonymous WWW User" even if the person completing the form has included his or her name and e-mail address.

This is how to program "choose one of the following" options using radio buttons where one of the choices must be selected (e.g. one cannot leave this question unanswered). The default answer will be the button that is already checked unless this is changed by the person completing the form.

This is how to program "choose all that apply" options using "checkboxes":
(However, while more than one selection will display on screen, 'Mailaa' has been programmed only to send one response. Thus "checkboxes" should not be used, as multiple selections will not actually be sent even if it appears on screen that they will be)

(The box will scroll to accommodate as much text as is entered. You may define the box as being wider, narrower, higher, lower, etc, if you anticipate greater or lesser amounts of feedback being entered).
